If you need to have a copy of our medical records relating to a patient's care forwarded to another physician or healthcare facility, we are happy to cooperate with your request. We need to receive a letter signed by the patient or legally authorized representative of the patient indicating the request to transfer medical records. This request letter must clearly identify the patient with full name, date of birth, and social security number. In addition, we require payment in advance for the cost of copying and sending the record.Our standard medical records processing fee is $1 per page up to a maximum of $100 with a minimum charge of $10. Payment should be made to Mercer Internal Medicine, LLC. |
