To refill a prescription, please ask your pharmacy to fax a refill request to our fax number at (609) 737-2427. Your full name, date of birth, medication details, physician name, and pharmacy fax number must be clearly stated on the request for us to properly process the prescription refill.If it has been longer than 6 months since your last visit with us, an office appointment for reevaluation may be required before a refill is authorized.Any new prescription for a medication not previously prescribed by our practice requires a medical appointment for a proper evaluation of your condition and treatment.Thanks for helping us to do a good job of caring for your health! |
